Minimum Qualifications

• Bachelor's degree in Social Work or Human Services
• State-required certifications or license
• 1–3 years supervised social work experience in a direct healthcare setting (preferred)
• Additional certifications preferred (e.g., Geriatric Case Management, Hospice & Palliative Care, Clinical Social Work, Substance Use)

Core Responsibilities

• Plan and implement a comprehensive Social Services program in compliance with federal and state regulations
• Apply departmental policies and procedures; participate in Quality Assurance and Performance Improvement activities
• Maintain accurate, timely patient/resident records: psychosocial assessments, care plans, progress notes
• Conduct comprehensive psychosocial assessments, discharge planning, and screening for trauma, mental illness, and intellectual disability
• Provide therapeutic interventions to support adjustment and facilitate group sessions for patients/residents and families
• Promote and protect patients’/residents’ rights and psychosocial well-being; identify and report abuse; participate in grievance investigations
• Coordinate transfers within the center and collaborate on discharge planning and communication with patients/residents, families, staff, and community providers
• Provide crisis interventions and staff support during catastrophic events; arrange and lead family meetings
• Educate staff on Social Services’ role and patients’ psychosocial needs
• May supervise Social Service students
• Perform other duties as assigned by Management
