Requirements:
- Active CMT, CNA/M, or LPN/LVN certification/license
- Experience in Long-Term Care or Skilled Nursing (preferred)
- Strong attention to detail and medication safety practices
- Ability to work independently and as part of a team
- High level of integrity, dependability, and compassion
- Passion for caring for long-term care residents and skilled rehabilitation patients
Responsibilities:
- Administer medications via approved routes under RN supervision
- Maintain medication carts and secure, organized storage areas
- Obtain approval before administering PRN medications
- Perform non-narcotic medication counts at shift change
- Monitor medication supplies and coordinate reorders
- Accurately document medications on the MAR
- Obtain and record vital signs, including apical pulse
- Observe and report changes in resident condition
- Assist with resident care, including CNA duties
- Communicate effectively with nursing staff, providers, and care teams
- Maintain professionalism with residents and families
- Work efficiently in a fast-paced environment with frequent interruptions
