Requirements:

• Active LPN/LVN license in Wisconsin (or practicing state as applicable).
• Prior experience in long-term care, skilled nursing, or assisted living preferred.
• Competence in medication administration, basic treatments, and ADL assistance.
• Strong clinical assessment, documentation, communication, and teamwork skills.
• Ability to follow care plans and report changes to RN/physician.

Core Responsibilities:

• Provide direct resident care, including assistance with activities of daily living.
• Administer medications and treatments per facility policy and under appropriate supervision.
• Monitor and document resident condition, vital signs, and care provided; report clinical changes promptly.
• Follow and implement individualized care plans and physician/RN orders.
• Maintain a safe, clean environment and adhere to infection control and safety protocols.
• Offer emotional support and companionship to residents; communicate with family and team as needed.
