Travel Registered Nurse (RN) – Home Health, St. Albans, VT (Contract)

We're looking for a compassionate, skilled RN to join a home health team in St. Albans, VT on a contract/travel basis starting ASAP. This role averages a 37-hour week with a pay rate of $2,501.

What you’ll bring

• Current RN license in Vermont.
• 1–2 years of nursing experience (home health experience is a plus).
• Strong clinical assessment and critical-thinking abilities.
• Clear communication skills and the ability to work independently while managing multiple patient visits.
• Valid driver’s license and reliable transportation.
• BLS certification required; other certifications a bonus.

What you’ll do

• Perform thorough patient assessments, create individualized care plans, and evaluate outcomes.
• Deliver nursing care including medication administration, wound care, and chronic disease management.
• Teach patients and families about disease management, medications, lifestyle changes, and self-care techniques.
• Monitor and document patient progress, report changes to physicians, and coordinate with the interdisciplinary team.
• Follow infection control, safety, and regulatory standards.
• Provide emotional support and guidance to patients and families about home care resources.
• Take part in quality improvement efforts and ongoing professional development.
