Requirements:

• Active RN license in practicing state
• BLS required
• Hospice or palliative care certification preferred
• Experience in home health, hospice, or palliative care preferred
• Strong clinical assessment, communication, patient-education, and documentation skills

Core Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans
• Safely administer medications, treatments, and nursing interventions
• Provide patient and family education and emotional support related to disease process and care goals
• Monitor patient status, identify changes, and promptly communicate with the interdisciplinary care team
• Accurately document care and maintain compliance with regulatory and quality standards
