Hospice RN Case Manager — Contract/Travel (Barre, VT)

We’re looking for an experienced RN to join a hospice team in Barre, VT on a contract/travel basis. This role is Monday–Friday, 36 hours per week, starting ASAP. Pay is $2,308 per 36-hour week.

What you’ll be doing

• Conduct thorough assessments and develop, implement, and evaluate individualized hospice care plans.
• Monitor patients’ conditions and adjust care as needed.
• Work closely with physicians, nurses, social workers, and other team members to coordinate care.
• Provide compassionate education and support to patients and families about disease progression and end-of-life options.
• Maintain accurate documentation that meets hospice standards and regulatory requirements.
• Participate in interdisciplinary team meetings and help drive quality improvement efforts.

What we’re looking for

• Active RN license in Vermont (or the state of practice).
• 1–2 years of nursing experience, ideally in hospice, palliative care, or home health.
• Strong clinical assessment skills, solid care planning experience, and excellent communication.
• Familiarity with hospice regulations, standards, and documentation expectations.
