Requirements:
- Active RN license in the state of practice.
- Minimum of 1–2 years of nursing experience, preferably in hospice, palliative care, or home health.
- Strong assessment, clinical, and communication skills.
- Knowledge of hospice regulations, standards, and care planning.
Responsibilities:
- Assess, plan, implement, and evaluate individualized hospice care for patients.
- Coordinate care with physicians, nurses, social workers, and other healthcare professionals.
- Provide education and support to patients and their families regarding disease progression and end-of-life care.
- Ensure documentation complies with hospice standards and regulatory requirements.
- Monitor patient conditions and adjust care plans as necessary.
- Participate in interdisciplinary team meetings and contribute to quality improvement initiatives.
