Requirements

• Active LPN license in the practicing state
• BLS certification

Core Responsibilities

• Provide direct nursing care under RN/physician supervision: medication administration, wound care, vital signs, and basic treatments.
• Perform patient assessments (physical exam, medical history, medication review) and contribute to individualized care plans.
• Administer medications and treatments accurately per orders and facility protocols.
• Evaluate, clean, dress, and monitor wounds and incisions for infection or complications.
• Monitor and document vital signs and changes in patient condition.
• Maintain accurate patient records, including treatments, medications, and observations.
• Educate patients and families on diagnoses, medications, self-care, and preventive measures.
• Administer and monitor IV medications and fluids; ensure proper IV line maintenance.
• Provide catheter care (insertion as authorized, maintenance, monitoring for complications).
• Collect specimens (blood, urine, sputum) for laboratory testing.
• Collaborate with the interdisciplinary team (RNs, physicians, therapists, support staff) to coordinate care.
• Assist with patient comfort, mobility, positioning, and pain management.
• Respond to emergencies per protocol and provide basic life support when needed.
• Follow infection control practices and proper PPE use.
• Handle, store, and dispose of medications and supplies per regulations.
• Advocate for patient rights and needs.
• Supervise CNAs/UAPs when assigned.
• Complete other duties as assigned by management.
