Required:

• Active Licensed Practical Nurse (LPN) state license
• BLS certification

Primary responsibilities:

• Provide direct nursing care under RN/physician direction: medication administration, wound care, vital sign monitoring.
• Perform focused patient assessments (physical exam, history, medication review) and contribute to individualized care plans.
• Administer medications and treatments accurately per orders and protocols; document administration.
• Assess, clean, dress, and monitor wounds/incisions for infection or complications.
• Monitor and record vital signs (BP, pulse, temperature, respiration) and report changes.
• Maintain accurate patient records: condition changes, treatments, and medication records.
• Educate patients and families on diagnoses, medications, self-care, and preventive measures.
• Administer and monitor IV medications/fluids; ensure IV line integrity.
• Provide catheter care (insertion, maintenance, monitoring) and identify complications.
• Collect specimens (blood, urine, sputum) for laboratory testing.
• Collaborate with RNs, physicians, therapists, and support staff to coordinate care.
• Assist with patient mobility, positioning, comfort, and basic pain management.
• Respond to emergencies following institutional protocols and provide basic life support as needed.
• Follow strict infection control practices and proper use of PPE.
• Handle, store, and dispose of medications and supplies according to policy and regulations.
• Advocate for patient rights and participate in interdisciplinary care planning.
• Supervise CNAs/UAPs when assigned to ensure quality care.
• Participate in ongoing education to maintain competency and follow best practices.
• Other duties as assigned by management.
