Rate: Travel $1,628/35 hrs; Local $1,603/35 hrs
Required Qualifications
- Active, valid Occupational Therapy state license
- CPR and BLS certification from AHA or Red Cross (with AED)
Key Responsibilities
- Perform evaluations of patients’ physical and cognitive abilities and assess home/environmental factors to identify limitations and set measurable goals
- Develop individualized treatment plans in collaboration with patients and the interdisciplinary team
- Implement therapeutic interventions to improve activities of daily living (ADLs), instrumental ADLs, functional mobility, cognition, and independence
- Recommend, train in, and document use of adaptive equipment and assistive devices
- Instruct and practice ADLs with patients; progress and modify tasks to promote independence
- Educate patients and families on condition management, safety, and strategies to maintain gains
- Monitor, document, and revise treatment plans based on patient response and outcomes
- Communicate and coordinate with physicians, nurses, therapists, and other care team members
- Maintain accurate clinical documentation including evaluations, treatment notes, and discharge summaries
- Support patient discharge planning and community/work reintegration
- Participate in ongoing professional development and remain current with OT best practices
- Perform other duties as assigned by management
