Required Qualifications

• Active, valid Occupational Therapy state license
• CPR and BLS certification from AHA or Red Cross (with AED)

Key Responsibilities

• Perform evaluations of patients’ physical and cognitive abilities and assess home/environmental factors to identify limitations and set measurable goals
• Develop individualized treatment plans in collaboration with patients and the interdisciplinary team
• Implement therapeutic interventions to improve activities of daily living (ADLs), instrumental ADLs, functional mobility, cognition, and independence
• Recommend, train in, and document use of adaptive equipment and assistive devices
• Instruct and practice ADLs with patients; progress and modify tasks to promote independence
• Educate patients and families on condition management, safety, and strategies to maintain gains
• Monitor, document, and revise treatment plans based on patient response and outcomes
• Communicate and coordinate with physicians, nurses, therapists, and other care team members
• Maintain accurate clinical documentation including evaluations, treatment notes, and discharge summaries
• Support patient discharge planning and community/work reintegration
• Participate in ongoing professional development and remain current with OT best practices
• Perform other duties as assigned by management
