Coverage type: Scheduled clinical hours
Practice setting: Outpatient community health center
Schedule: Monday _ Friday, 8:00 AM _ 5:00 PM
Patient volume: Minimum 18 patients per day
EMR: Epic
Credentialing timeframe: approximately 30 _ 90 days
Decision timeframe: approximately 7 _ 14 days
Providers requested: 1

Clinical Responsibilities
Outpatient family medicine evaluation and treatment
Care for mixed adult and pediatric patient population
Preventive care and chronic disease management
Well woman care and IUD placement preferred
Some chronic pain management
Routine follow ups and continuity of care
Timely chart completion within 72 hours

MA support: 2

Requirements
Board Eligible or Board Certified in Family Medicine or Internal Medicine
MD or DO accepted
Active California license required
Active DEA required prior to start
BLS certification required
Current immunizations and titers required for credentialing
All employment gaps on CV must be accounted for
