- Active ID License or IMLC
- BC in Critical Care
- Fellowship in Critical Care
- DEA
- CSR/CDS
- ACLS
- Case logs
- NPDB report (pulled within the last month)
- Health Immunization
- Proof of COVID vaccination
- Comfortable performing procedures
- Must be able to cover most dates (not 1 or 2 blocks)
