- Valid driver’s license and reliable transportation
- Strong assessment, time management, and communication skills
Responsibilities:
- Provide skilled nursing care in patient homes or community settings
- Perform assessments and develop individualized care plans
- Administer medications, IV therapy, and treatments as ordered
- Perform wound care and monitor patient progress
- Educate patients and caregivers on care and disease management
- Coordinate care with physicians and interdisciplinary teams
- Document visits and patient status in the EMR
- Travel between patient locations as required
