Contract/Travel Registered Nurse — Case Manager | Glenwood Springs, CO
Start: ASAP | Shift: 36-hour assignment | Rate: $2,434/36 hrs

Overview:
Join a multidisciplinary team as a case manager providing clinical coordination and utilization review for patients in the Glenwood Springs area. This contract role emphasizes patient-centered care planning, cross-setting coordination, and quality-focused documentation.

Qualifications:

  • Current, active RN license in the practicing state
  • Experience in case management, utilization review, or clinical coordination preferred
  • Strong organizational, clinical judgment, and communication skills
  • BLS required; CCM or specialty case management certification preferred

Key Responsibilities:

  • Conduct comprehensive assessments and develop individualized care plans
  • Coordinate services across multidisciplinary teams and healthcare settings to support safe transitions of care
  • Monitor patient progress, revise plans as needed, and track outcomes against quality metrics
  • Provide education and resource guidance to patients and families
  • Maintain accurate documentation and ensure compliance with regulatory and institutional standards

Compensation and logistics:

  • Contract/travel assignment beginning ASAP in Glenwood Springs, CO
  • Pay: $2,434 per 36-hour period
