Remote RN Case Manager — Diabetic Care (Contract, ASAP Start, Travel Pay)

Rate: Travel $2,028/40 hrs | Local $1,952/40 hrs

Core responsibilities

• Provide remote case management for diabetic patients.
• Conduct virtual assessments, monitor patient progress, and perform ongoing follow-up.
• Develop, implement, and revise individualized care plans.
• Coordinate care with multidisciplinary teams to optimize outcomes.
• Document all interactions, assessments, and plan changes per facility standards.
• Ensure compliance with state regulations, professional guidelines, and organizational policies.
• Maintain patient confidentiality and data security in a remote setting.

Required qualifications

• Active New York RN license.
• Nurse Compact / multi-state license (required).
• Minimum 3 years of diabetic care experience.
• Clinical experience in care coordination or case management preferred.
• Strong communication, critical thinking, and organizational skills.
• Must provide own technology for remote work (computer, webcam, microphone, speakers, phone, and stable internet).
