RN Case Manager (Diabetes) — Remote/Contract, Fort Washington PA

Join a remote case management team providing diabetic care for patients in Fort Washington, PA. This is a 4-week contract/travel assignment starting ASAP, scheduled 8 hours/day, 40 hours/week. Travel pay: $2,028/40 hrs; Local pay: $1,952/40 hrs.

Key requirements

• Active New York RN license and Nurse Compact/multi-state license required
• Minimum 3+ years’ experience in diabetic care
• Clinical experience in care coordination or case management preferred
• Must supply personal technology (computer, webcam, microphone, speakers, phone) and stable internet
• Strong communication, critical thinking, and organizational skills

Primary responsibilities

• Deliver remote case management and virtual assessments for diabetic patients
• Monitor patients continuously and adjust care plans to meet changing needs
• Create, implement, and update individualized care plans
• Coordinate with multidisciplinary teams to improve outcomes
• Accurately document all interactions and maintain records per facility standards
• Ensure compliance with state regulations, professional guidelines, and organizational policies
• Protect patient confidentiality and secure data in the remote setting
