Assignment: Contract/Travel — Start ASAP — Duration: 4 weeks
Schedule: 8 hours/day, 40 hours/week
Pay: Travel: $2,028 per 40 hours | Local: $1,952 per 40 hours

Position overview:
Provide remote case management for patients with diabetes. Perform virtual assessments, develop and revise individualized care plans, coordinate with multidisciplinary teams, and document all care in accordance with facility and regulatory standards.

Key responsibilities:

• Conduct virtual assessments and ongoing monitoring of diabetic patients.
• Develop, implement, and modify individualized care plans based on assessments and clinical need.
• Coordinate care across disciplines to optimize patient outcomes.
• Document all patient interactions, care plans, and updates per facility standards.
• Ensure compliance with state regulations, professional guidelines, and organizational policies.
• Maintain patient confidentiality and data security in a remote setting.

Required qualifications:

• Active New York RN license.
• Nurse Compact / Multi-state license (required).
• Minimum 3 years’ experience in diabetic care.
• Clinical experience in care coordination or case management preferred.
• Strong communication, critical thinking, and organizational skills.
• Ability to provide your own technology: computer, webcam, microphone, speakers, phone, and stable internet connection.
