RN Case Manager – Remote Diabetic Care (Contract, Fort Washington PA)

We’re seeking a Registered Nurse to provide remote case management for patients with diabetes. This is a contract/travel opportunity based in Fort Washington, PA, starting ASAP for a 4-week assignment. You’ll work 8-hour shifts, 40 hours per week, delivering virtual assessments, ongoing monitoring, and coordinated care planning alongside a multidisciplinary team.

• Rate: Travel: $2,028/40 hours • Local: $1,952/40 hours

What we’re looking for:

• Active New York RN license
• Nurse Compact / multi-state license (required)
• Minimum 3+ years’ experience in diabetic care
• Clinical experience in care coordination or case management preferred
• Strong communication, critical thinking, and organizational skills
• Ability to maintain patient confidentiality and data security in a remote setting
• You must provide your own technology (computer, webcam, microphone, speakers, phone, and a stable internet connection)

Core responsibilities:

• Provide remote case management for diabetic patients, including virtual assessments and ongoing monitoring
• Create, implement, and adjust individualized care plans based on patient needs
• Collaborate with physicians, educators, and other care team members to optimize outcomes
• Document all interactions and plan updates per facility standards
• Ensure compliance with state regulations, professional guidelines, and organizational policies while working remotely
