We are seeking a compassionate and driven physician to join our post-acute care team in Port Charlotte, Florida rounding in a skilled nursing facility.

- Day call weekdays, weekends, after hours/night-time backup call
- Perform comprehensive patient assessments, including medical histories and physical exams
- Develop, implement, and adjust individualized care plans based on patient needs
- Monitor progress and coordinate care to optimize outcomes
- Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach
- Educate patients and families on health management and preventive strategies
- Maintain accurate, timely medical records in compliance with regulations

**Qualifications**
- Current Florida physician license and DEA
- Board certification/board eligible in family medicine or internal medicine
- Experience in post-acute, acute, or primary care clinic settings preferred, but not required; new Residency grads welcome to apply
- Strong clinical foundation and knowledge of healthcare regulatory standards
- Excellent communication and interpersonal skills
- Collaborative, team-oriented approach with a positive outlook
- Ability to Commute: Port Charlotte, FL (required)
- Work Location: In person

**Compensation & Benefits**
- Our compensation estimate for the position is $235,212 to $278,254 annually with no cap on productivity income potential.
