**Nurse Practitioner – Internal Medicine**
**Location: Peterborough, NH**
• Provide primary care services to patients across multiple clinical sites in a community-based setting
• Collaborate closely with the Medical Director and Director of Medical Practice Operations to ensure high-quality, coordinated care
• Serve as a key clinical provider within a network of primary care practices, supporting accessible and value-based healthcare delivery
• Deliver patient-centered care through comprehensive assessments and development of individualized treatment plans
• Diagnose and manage acute and chronic conditions, including prescribing medications and therapies as appropriate
• Promote preventive healthcare through screenings, wellness visits, and patient education initiatives
• Maintain accurate, timely, and compliant electronic health records (EHR documentation)
• Work collaboratively with physicians, nurses, and interdisciplinary teams to optimize patient outcomes
• Participate in quality improvement initiatives and clinical best practice development
• Support a culture grounded in compassion, collaboration, honesty, and respect
• Contribute to a team-oriented, community-focused healthcare environment
• Perform additional duties as assigned
