Requirements:
- Active LVN/LPN license in the practicing state
- Graduate of an accredited LVN/LPN program
- BLS certification
- Skilled Nursing or Long-Term Care experience (preferred)
- Knowledge of state nursing regulations and SNF protocols
- Ability to lift, reposition, and assist residents as needed
Responsibilities:
- Provide direct patient care according to care plans and physician orders
- Administer medications and treatments as prescribed
- Monitor and document patient conditions, vital signs, and changes in status
- Assist with admissions, discharges, and transfers
- Perform wound care and assist with rehabilitative services
- Collaborate with RNs, physicians, and interdisciplinary care teams
- Educate residents and families on care plans and treatments
- Maintain accurate and timely documentation
- Follow facility policies, state regulations, and infection control standards
