**About the Position**
Seeking a Family Practice Physician to join an outpatient team in Crestview, Florida. This role involves diagnosing and treating a wide variety of conditions and injuries in a general practice setting, caring for patients of all ages. The physician will work collaboratively with the care team as well as local, regional, state, and national healthcare resources to coordinate patient care and services.

**Schedule**
-Monday through Friday
-Typically 8:00 a.m. to 5:00 p.m.
-No holidays
-No weekends

**Qualifications**
-MD or DO required
-Completion of Family Practice specialization
-Current Florida medical license required
-Current DEA certificate required
-Board eligible or board certified in Family Practice
-If board eligible at time of hire, must become board certified within 3 years of hire date
-Must be insurable for malpractice coverage
-Current CPR certification required
