Seeking two (2) experienced Advanced Practice Providers (APPs) - one to join our clinic in Fairplay and one to join our clinic in Westcliffe. Both positions are full-time, clinic-based roles serving rural communities with a strong need for accessible, high-quality primary care.
These are two opportunities to practice meaningful medicine in a setting where you can build relationships, work collaboratively and truly impact the health of the community.

Key Responsibilities
Full-time, outpatient primary care
Practice independently with strong physician support available
Broad scope across the lifespan (pediatrics to geriatrics)
Opportunities to manage chronic conditions, same-day needs and preventative care
Supportive clinic teams with Medical Assistants (MAs) and operational leadership in place

Qualifications - What we're looking for:
Licensed Family Nurse Practitioner (FNP) or Physician Assistant
Minimum of 2+ years of primary care experience required
Comfortable practicing in a rural setting with a wide scope
Strong clinical judgment and ability to work independently
Team-oriented with a patient-centered approach
