Position: Lead Care Management Coordinator (LVN)
Location: Clearlake, California
Duration: 13 Weeks | Start: ASAP

Pay Package:

• Pay Rate: $16/hr on W2 + $700 Weekly Stipend for meals and lodgings.

Job Overview:

We are seeking a Lead Care Management Coordinator (LVN) to manage clinical intake processes, coordinate referrals, and ensure smooth communication between patients, families, and healthcare teams. This role involves supervising workflow, verifying insurance, and supporting home health scheduling and coordination.

Requirements:

• Active LVN (Licensed Vocational Nurse) Certification – Required
• Minimum 1 Year of Experience – Required
• Clinical Intake Coordinator Experience – Required
• Strong communication and coordination skills
• Experience in home health processing & referral scheduling – Preferred

Shift:

• Day Shift: Monday – Friday
• 8:00 AM – 5:00 PM (5x8s)
• 40 Hours per Week
• No On-Call
