• Monday - Friday 8am - 4:30pm, no weekends, no call
• Focus on heart failure patients with aim of reducing readmissions
• Hospital and clinic setting with IP/OP duties
• Cardiology consultations, follow-ups, and discharge coordination
• Medically complex adult patient demographic
• Several years of heart failure experience required
• ACLS and BLS certification required
• Hospital privileges required
