- Ability to work independently in community settings
Responsibilities:
- Provide nursing care to individuals with intellectual and developmental disabilities
- Perform health assessments and monitor chronic conditions
- Develop and implement individualized care plans
- Administer medications and treatments as ordered
- Coordinate care with families, caregivers, and community providers
- Educate patients and caregivers on health management
- Support community-based health programs and services
- Document care and patient interactions in the EMR
