Required credentials

• Valid, active Licensed Practical Nurse (LPN) state license
• BLS certification

Core responsibilities

• Provide direct nursing care per RN/physician orders: medication administration, wound care, vital signs, and basic treatments.
• Perform patient assessments (physical exam, history, medication review) and contribute to individualized care plans.
• Administer prescribed medications and treatments accurately and in accordance with protocols.
• Assess, clean, dress, and monitor wounds/incisions for infection or complications.
• Monitor and document vital signs (BP, pulse, temperature, respirations) and report changes in condition.
• Maintain accurate patient records of assessments, treatments, medications, and condition changes.
• Educate patients and families on diagnoses, medications, self-care, and preventive health measures.
• Administer and monitor IV medications/fluids; manage IV lines per facility policy.
• Provide catheter care (insertion, maintenance, monitoring) and identify complications.
• Collect specimens (blood, urine, sputum) for lab testing following proper procedures.
• Assist with patient mobility, positioning, comfort, and pain management.
• Respond to medical emergencies and provide basic life support as indicated.
• Follow infection control policies, including hand hygiene and PPE use.
• Handle, store, and dispose of medications and supplies safely and per regulations.
• Collaborate with RNs, physicians, therapists, CNAs, and support staff to coordinate care.
• Supervise CNAs/UAPs when assigned to ensure quality patient care.
• Maintain professional competency through ongoing education and training.
• Perform other duties as assigned by management.
