Required credentials
- Valid, active Licensed Practical Nurse (LPN) state license
- BLS certification
Core responsibilities
- Provide direct nursing care per RN/physician orders: medication administration, wound care, vital signs, and basic treatments.
- Perform patient assessments (physical exam, history, medication review) and contribute to individualized care plans.
- Administer prescribed medications and treatments accurately and in accordance with protocols.
- Assess, clean, dress, and monitor wounds/incisions for infection or complications.
- Monitor and document vital signs (BP, pulse, temperature, respirations) and report changes in condition.
- Maintain accurate patient records of assessments, treatments, medications, and condition changes.
- Educate patients and families on diagnoses, medications, self-care, and preventive health measures.
- Administer and monitor IV medications/fluids; manage IV lines per facility policy.
- Provide catheter care (insertion, maintenance, monitoring) and identify complications.
- Collect specimens (blood, urine, sputum) for lab testing following proper procedures.
- Assist with patient mobility, positioning, comfort, and pain management.
- Respond to medical emergencies and provide basic life support as indicated.
- Follow infection control policies, including hand hygiene and PPE use.
- Handle, store, and dispose of medications and supplies safely and per regulations.
- Collaborate with RNs, physicians, therapists, CNAs, and support staff to coordinate care.
- Supervise CNAs/UAPs when assigned to ensure quality patient care.
- Maintain professional competency through ongoing education and training.
- Perform other duties as assigned by management.
