Licensed Practical Nurse (LPN/LVN) — Long Term Care, Uniontown PA (12Week Contract, PM Shift)

We’re looking for an experienced Licensed Practical Nurse to join a team in Uniontown, PA for a 12-week contract starting ASAP. This is a local, non-travel position on the 3:00 PM–11:30 PM shift, paid at $1,264 per 40-hour week.

What you’ll need

• Current, active Licensed Practical Nurse state license
• BLS (Basic Life Support) certification

What you’ll do
You’ll provide hands-on nursing care under the direction of RNs and physicians, focusing on patient comfort, safety, and positive outcomes. Typical duties include:

• Administer medications and treatments accurately and according to protocols
• Perform patient assessments (physical exams, histories, medication reviews) and help develop individualized care plans
• Monitor and document vital signs and changes in patient condition
• Provide wound care: evaluate, clean, dress, and monitor for infection or complications
• Start, maintain, and monitor IVs and IV medications when required
• Manage urinary catheters, including insertion, routine care, and complication monitoring
• Collect specimens (blood, urine, sputum) for lab testing
• Keep detailed, accurate patient records of treatments and observations
• Educate patients and families about diagnoses, medications, and at-home care
• Assist with mobility, positioning, and pain management to promote comfort and safety
• Respond quickly in medical emergencies, following protocols and providing BLS as needed
• Follow strict infection control practices and use PPE appropriately
• Handle, store, and dispose of medications and supplies safely and according to policy
• Promote preventive care (vaccinations, screenings) and support health assessments
• Advocate for patients’ needs and coordinate with the healthcare team—RNs, physicians, therapists, and support staff—to ensure seamless care
• Supervise CNAs or unlicensed assistive personnel when required
• Participate in ongoing education to stay current with nursing best practices
• Other duties as assigned by management
