Required Credentials
- Active, valid Licensed Practical Nurse (LPN) state license
- BLS certification
Core Responsibilities
- Provide direct nursing care under RN/physician guidance: medication administration, wound care, vital signs, ADL assistance.
- Conduct patient assessments (physical exam, history, medication review) and contribute to individualized care plans.
- Administer prescribed medications and treatments accurately, following protocols.
- Manage wounds and incisions: cleaning, dressing, monitoring for infection/complications.
- Monitor and document vital signs and patient condition changes.
- Maintain accurate patient records of assessments, treatments, medications, and progress.
- Educate patients and families on conditions, medications, self-care, and preventive measures.
- Administer and monitor IV medications/fluids; ensure proper IV line function and care.
- Provide catheter care: insertion (if allowed by facility policy), maintenance, and complication monitoring.
- Collect specimens (blood, urine, sputum) for laboratory testing.
- Collaborate with RNs, physicians, therapists, and support staff to coordinate care.
- Address patient comfort and safety: positioning, mobility assistance, pain management.
- Respond to emergencies per protocols and provide basic life support as needed.
- Follow infection control practices (hand hygiene, PPE) and safe handling/storage/disposal of medications and supplies.
- Promote preventive care (vaccinations, screenings) and support health assessments.
- Advocate for patient rights and needs.
- Supervise CNAs/UAPs when applicable to ensure quality care.
- Perform other duties as assigned by management.
