LPN/LVN - Licensed Practical Nurse (12-Week Contract) — Long Term Care, Franklin NH

Join a Franklin, NH facility as a Licensed Practical Nurse for a 12-week, contract (non-travel) assignment starting ASAP. This evening/night shift runs 7:00 PM–7:30 AM. Compensation: Local pay $1,098/36 hours.

Requirements:

• Current, active LPN license for the state of New Hampshire
• BLS certification

Key responsibilities:

• Provide direct nursing care under RN/physician direction, including medication administration, wound care, IV therapy, and catheter care
• Conduct focused patient assessments (physical exams, histories, medication reconciliation) and implement individualized care plans
• Monitor, record, and report vital signs and changes in patient condition; maintain accurate clinical documentation
• Administer and evaluate prescribed medications and treatments, ensuring correct dosing and adherence to protocols
• Assess, clean, dress, and manage wounds; monitor for signs of infection or complications
• Collect specimens (blood, urine, sputum) for laboratory testing
• Assist with patient comfort, mobility, positioning, pain management, and basic ADLs as needed
• Provide patient and family education on diagnoses, medications, self-care, and preventive measures
• Respond to medical emergencies and provide basic life support per facility protocols
• Adhere to infection control practices and proper use of PPE; safely handle, store, and dispose of medications and supplies
• Collaborate with multidisciplinary team members (RNs, physicians, therapists, support staff) to coordinate care
• When delegated, supervise CNAs or unlicensed assistive personnel to ensure quality care
• Maintain professional competence through ongoing education and follow applicable regulations and facility policies
• Other duties as assigned by management
