Requirements:

• Active RN license in the practicing state.
• Inpatient nursing, case management, or care coordination experience preferred.
• BLS required.
• CCM or care coordination certification preferred.
• Strong clinical judgment, organizational ability, and written/verbal communication skills.

Responsibilities:

• Assess patient needs and develop individualized care plans.
• Coordinate care across multidisciplinary teams and hospital departments.
• Monitor patient progress and update care plans as needed.
• Educate patients and families on treatment plans and post-discharge care.
• Maintain accurate clinical documentation and ensure compliance with regulatory standards and quality metrics.
