Requirements:

• Active Registered Nurse license (state appropriate)
• Hospice care knowledge; familiarity with Medicare/Medicaid guidelines
• Strong assessment, documentation, and communication skills
• Ability to work independently in home-care settings
• IV therapy and venipuncture experience preferred
• Valid driver’s license, reliable vehicle, and insurance
• Ability to meet physical demands of home-based care
• Commitment to hospice philosophy and ethical nursing practice

Core Responsibilities:

• Respond to after-hours patient and family calls; assess needs and prioritize care
• Provide pain and symptom management per physician plan of care
• Perform ongoing patient assessments and accurately document interventions and outcomes
• Communicate patient status and changes with physicians and the interdisciplinary team
• Educate and support patients and caregivers regarding care plans and symptom management
• Coordinate referrals to hospice disciplines and community resources as needed
• Maintain patient confidentiality and comply with agency policies and regulations
• Work independently, triage urgent situations, and follow established care protocols
