Hospice Registered Nurse (RN) On-Call, Evenings (6-Week Contract, Fort Washington PA)

Join a hospice team in Fort Washington, PA, for a 6-week contract where you’ll provide compassionate after-hours care. This evening shift role starts ASAP and can be filled as travel ($771/32-hrs) or local ($739/32-hrs).

What you’ll do:

• Answer after-hours patient and family calls, quickly assessing needs and prioritizing urgent situations.
• Manage pain and symptoms according to the physician’s plan of care.
• Perform ongoing patient assessments in home settings and document care clearly and accurately.
• Communicate effectively with physicians and the interdisciplinary hospice team to ensure continuity of care.
• Educate and support patients and caregivers, helping them understand the plan and available resources.
• Coordinate referrals to hospice disciplines and community services as needed.
• Maintain patient confidentiality and follow agency policies while working independently in the field.

What we’re looking for:

• Active Registered Nurse license.
• Comfort and experience with hospice care and knowledge of Medicare/Medicaid guidelines.
• Strong assessment, documentation, and communication skills.
• Ability to work independently in home-care environments and handle the physical demands of home visits.
• Experience with IV therapy and venipuncture preferred.
• Valid driver’s license, reliable vehicle, and current insurance.
• A commitment to hospice philosophy and ethical nursing practice.
