Required qualifications

• Valid, active LPN license for the state of New Hampshire
• Current BLS certification

Core responsibilities

• Provide direct nursing care under RN/physician direction: medication administration, wound care, vital signs, and basic treatments.
• Perform focused patient assessments (physical exam, history, medication review) and contribute to individualized care plans.
• Safely administer medications and treatments, document doses, and follow protocols.
• Assess, clean, dress, and monitor wounds and surgical sites for infection/complications.
• Obtain and record vital signs and other clinical data; maintain accurate, timely patient records.
• Collect specimens (blood, urine, sputum) for lab testing.
• Monitor and manage IV therapy and urinary catheters per facility policy.
• Support patient comfort, mobility, positioning, and pain management.
• Educate patients and families on conditions, medications, and self-care.
• Respond to emergencies and provide basic life support as required.
• Follow infection control practices and PPE use; handle, store, and dispose of medications/supplies safely.
• Collaborate with RNs, physicians, therapists, and support staff to coordinate care.
• Promote preventive care and advocate for patient rights and needs.
• May supervise CNAs or unlicensed assistive personnel when assigned.
• Maintain professional competence through ongoing education and comply with facility policies.
• Other duties as assigned by management.
