Full-time position in Bangor, Maine — join a new outpatient primary care clinic to provide comprehensive, patient-centered care to individuals and families. Key responsibilities

• Deliver full-spectrum primary care across lifespan: prevention, wellness, acute visits, and chronic disease management
• Perform histories, physicals, diagnostic testing, treatment planning, and medication management
• Coordinate care with physicians, nurses, behavioral health, and community resources
• Document accurately in the EHR and ensure continuity of care
• Participate in quality improvement, clinic development, and patient education

Qualifications
