Job Description
Full-time position in Bangor, Maine — join a new outpatient primary care clinic to provide comprehensive, patient-centered care to individuals and families. Focus areas include preventive care, chronic disease management, and acute care. Opportunity to shape clinic workflows and contribute to program development.
Key responsibilities
- Deliver full-spectrum primary care across lifespan: prevention, wellness, acute visits, and chronic disease management
- Perform histories, physicals, diagnostic testing, treatment planning, and medication management
- Coordinate care with physicians, nurses, behavioral health, and community resources
- Document accurately in the EHR and ensure continuity of care
- Participate in quality improvement, clinic development, and patient education

Qualifications
- Licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) in Maine, or eligible to obtain licensure

- Ability to work collaboratively in a team-based setting
