Float Family Nurse Practitioner – Primary Care (West Phoenix, AZ)

Join our team in West Phoenix as a full-time Family Medicine Nurse Practitioner who thrives on variety and hands-on primary care. You’ll float across clinics and care settings, delivering patient-centered assessments and treatments, coordinating with other clinicians, and helping ensure smooth transitions and continuity of care for a diverse patient population.

What you’ll do:

• Provide comprehensive primary care across multiple clinical environments — from routine wellness exams to acute visits and chronic disease management.
• Perform thorough assessments, develop and implement treatment plans, and follow up to track patient progress.
• Collaborate closely with physicians, nurses, and ancillary staff to coordinate care and referrals.
• Educate patients and families about diagnosis, treatment options, preventive care, and self-management strategies.
• Adapt quickly to different clinic workflows and patient needs as a floating provider.

What we’re looking for:

• Master’s or Doctoral degree in Nursing (Family Nurse Practitioner focus).
• Current Family Nurse Practitioner certification.
• Valid Arizona RN/APRN licensure and active DEA registration.
• Experience in family medicine or primary care settings.
• Excellent communication and interpersonal skills, with the ability to build rapport across diverse populations.
• Flexibility and comfort working in multiple clinical locations and settings.
