Full-time position in West Phoenix, AZ providing comprehensive primary care to individuals and families across the lifespan. Responsibilities include preventive care, health maintenance, evaluation and management of acute and chronic medical conditions, and patient education to support self-management and wellness.

Key responsibilities

• Deliver routine primary care visits, acute care assessments, and chronic disease management
• Provide preventive services: screenings, immunizations, wellness counseling
• Develop and coordinate treatment plans, referrals, and follow-up care
• Educate patients and families on diagnoses, medications, lifestyle modifications, and care plans
• Maintain accurate medical records and participate in quality improvement initiatives

Qualifications

• MD or DO
• Board certified in Internal Medicine
• Active Arizona medical license and DEA registration
• Strong clinical, diagnostic, and communication skills
