Family Medicine Physician — Value-Based Advanced Primary Care (Gilbert, AZ)

We are hiring experienced Family Medicine physicians to join our Value-Based Advanced Primary Care team in Gilbert, AZ. This fully on-site role offers the opportunity to deliver comprehensive, patient-centered care within an integrated, team-based setting. Ideal candidates have a minimum of five years of clinical Family Practice or Med-Peds experience and a commitment to high-quality, coordinated primary care. Opportunities also available in Peoria, Phoenix, and Scottsdale, AZ.

Compensation & benefits

• Salary: $240,000–$265,000 (DOE)

Requirements

• MD or DO from an accredited medical school
• Board certified or board eligible in Family Medicine or Med-Peds
• Prefer minimum 5 years of clinical experience
• Current, unrestricted Arizona medical license; active DEA registration
• Arizona fingerprint clearance card
• BLS certification
• Proficiency with EMR systems and basic computer skills
• Strong communication, interpersonal, and teamwork abilities
• Experience or willingness to work within value-based, integrated care models
• Outstanding patient service and professionalism
