**Post-Acute Care Opening in IL**
Located in Dolton, Illinois - immediately south of the city limits of Chicago.
**About the Position**
A Primary Care Physician is sought to join a post-acute care team providing services in skilled nursing facilities. This full-time opportunity offers strong clinical support, autonomy, and excellent earning potential within a nationally recognized healthcare organization.
-Full-time schedule (5 days/week)
-Provide care within skilled nursing facilities
-Perform comprehensive patient assessments and care planning
-Manage and coordinate post-acute patient care
-Collaborate with multidisciplinary healthcare teams
-Educate patients and families on health management and preventive care
-New graduates encouraged to apply

-Post-acute care environment with strong operational support
-Comprehensive 3-month onboarding and mentorship program
-Dedicated clinical field mentor and leadership support
-Participation in TeamACO Medicare Shared Savings Program

**Compensation and Benefits**
-Estimated compensation: $229K–$270K annually
