**Post-Acute Care Opening in MI**
Located in Macomb, Michigan - Detroit 34m.
**About the Position**
A Post-Acute Care Physician is sought to join a skilled nursing facility-based practice within a nationally recognized healthcare organization. This full-time opportunity offers strong support, autonomy, and uncapped earning potential in a collaborative post-acute environment.
-Full-time schedule (5 days/week)
-Provide care within skilled nursing facilities
-Perform comprehensive patient assessments and develop individualized care plans
-Monitor patient progress and coordinate multidisciplinary care
-Collaborate closely with physicians, nursing staff, and healthcare professionals
-Educate patients and families on treatment and preventive care
-New graduates encouraged to apply
**Practice Setting**
-Post-acute care practice with extensive onboarding and mentorship
-Comprehensive 3-month onboarding program
-Dedicated clinical mentor and leadership support
-Participation in TeamACO Medicare Shared Savings Program
