**Post-Acute Care Medical Director Opening in NY**
Located in Cortland, New York - Near Syracuse
**About the Position**
A physician is sought to join a post-acute care team providing skilled nursing facility care while serving in a medical directorship role. This opportunity offers leadership responsibilities, schedule flexibility, and strong earning potential within a supportive clinical environment.
-Full-time schedule (4–5 days/week)
-Provide rounding services in skilled nursing facilities
-Medical directorship responsibilities with additional stipend income
-Perform comprehensive patient assessments and care planning
-Monitor patient progress and coordinate multidisciplinary care
-Educate patients and families on treatment and preventive strategies
**Practice Setting**
-Post-acute care model with extensive operational and clinical support
-Comprehensive 3-month onboarding and mentorship program
-Dedicated clinical field mentor and leadership guidance
-Participation in TeamACO Medicare Shared Savings Program
-Access to population health and quality improvement resources
-Medical Director Leadership Academy for leadership development
**Compensation and Benefits**
-Estimated compensation: $232K–$264K annually
-Uncapped productivity earning potential

-MD or DO
