**Post-Acute Care Opening in TX**
Located in Katy, Texas - 30 minutes away from Houston
**About the Position**
A Primary Care Physician is sought to join a post-acute care team providing services in skilled nursing facilities throughout the Katy and West Houston region. This full-time opportunity offers autonomy, strong support, and uncapped earning potential within a collaborative care model.
-Full-time schedule (5 days/week)
-Provide care within skilled nursing facilities
-Service area includes Katy, Sugar Land, Missouri City, Richmond, and West Houston
-Perform comprehensive patient assessments and care planning
-Monitor patient progress and coordinate multidisciplinary care
-Educate patients and families on treatment and preventive strategies
-New graduates encouraged to apply
**Practice Setting**
-Post-acute care practice with strong clinical and operational support
-Comprehensive 3-month onboarding and mentorship program
-Dedicated clinical field mentor and leadership support
-Participation in TeamACO Medicare Shared Savings Program
