Job Description
- Board Certified Internal Medicine or Family Medicine or Geriatric Medicine
- New office location in Lady Lake, FL with an existing patient panel
- Lower daily patient volume focused on comprehensive patient care
- Outpatient-only schedule with no nights, weekends, holidays, or hospital responsibilities
- Monday"Friday schedule
- Provide care for the senior patient populations
- Focus on prevention, quality outcomes, and evidence-based medicine
- Manage acute and chronic conditions including diabetes, COPD, CHF, obesity, and cardiovascular disease
- Team-based support including care coordination, pharmacy resources, and data tools
- Opportunity to supervise Nurse Practitioners and Physician Assistants
- Established patient base in a managed care setting
- EMR-based practice environment
- Seeking physicians with strong diagnostic and chronic disease management experience
- Active Florida medical license
- Minimum 3 years of outpatient primary care experience preferred
