Required qualifications

• Active, valid Licensed Practical Nurse (LPN) state license
• BLS certification

Core responsibilities

• Provide direct nursing care under RN/physician direction: medication administration, wound care, IV care, vital signs, and patient assessments.
• Perform physical assessments, obtain medical histories and medication reconciliations to support individualized care plans.
• Administer medications and treatments accurately, following orders and protocols.
• Manage wounds: evaluate, clean, dress, and monitor for infection or complications.
• Monitor and document vital signs and changes in patient condition; maintain accurate patient records.
• Insert, monitor and maintain IV lines; administer and observe IV medications/fluids per policy.
• Care for urinary catheters: insertion, maintenance, and monitoring for complications.
• Collect specimens (blood, urine, sputum) for laboratory testing.
• Educate patients and families on diagnoses, medications, self-care, and preventive measures.
• Assist with patient mobility, positioning, comfort, and pain management.
• Respond to emergencies and provide basic life support as needed.
• Follow infection control procedures and proper PPE use.
• Handle, store, and dispose of medications and supplies safely and in compliance with regulations.
• Collaborate with the interdisciplinary team (RNs, physicians, therapists, support staff) to coordinate care.
• Advocate for patient rights and needs.
• Supervise CNAs or UAPs when assigned.
• Maintain professional development and comply with current nursing standards.
• Perform other duties as assigned by management.
