• Monday - Thursday 8am - 5pm, Friday 8am - 12pm
• 15 patients per day
• Long term care experience required
• Geriatric experience required
• SNF experience preferred
• Palliative care skills preferred
• 3 weeks with possibility to extend to 8 weeks
• Geriatric patients, ages 65+
