Hospice Case Manager RN (Contract/Travel) — Barre, VT — 36-hr Week, ASAP Start

We’re looking for an experienced RN to join a hospice team in Barre, VT on a contract/travel basis starting ASAP. This is a Monday–Friday role with a 36-hour week and a pay rate of $2,308 per 36 hours.

What you’ll bring:

• An active RN license in the practicing state.
• At least 1–2 years of nursing experience, ideally in hospice, palliative care, or home health.
• Strong assessment and clinical skills, excellent communication, and comfort working with patients and families at end of life.
• Familiarity with hospice regulations, standards, and care planning.

What you’ll be doing:

• Conduct thorough assessments and develop, implement, and evaluate individualized hospice care plans.
• Coordinate patient care with physicians, nurses, social workers, and other members of the care team.
• Provide education and emotional support to patients and families about disease progression and end-of-life choices.
• Maintain accurate documentation that meets hospice standards and regulatory requirements.
• Monitor changes in patient condition and update care plans as needed.
• Participate in interdisciplinary team meetings and help drive quality improvement efforts.
