Job Description
Position: Integrated Case Manager
Location: Detroit, MI (Onsite)
Duration: 13-week contract
Schedule:
- Monday – Friday, 8:00 AM – 4:30 PM
- Weekend rotation required
Position Overview:
This role focuses on coordinating patient care, managing discharge planning, and ensuring effective utilization of healthcare services. Due to limited onsite training, candidates must be able to adapt quickly and work independently in a fast-paced environment.
Key Responsibilities:
- Coordinate patient care across the continuum
- Lead discharge planning and ensure safe transitions of care
- Collaborate with interdisciplinary teams including physicians and nursing staff
- Assess patient needs and develop care plans
- Ensure compliance with healthcare guidelines and utilization standards
- Maintain accurate documentation
Required Qualifications:
- 5+ years of case management experience
- 2+ years of recent acute inpatient case management experience
- BSN or MSW
- Current Michigan RN license or LMSW
- Strong discharge planning experience
- Ability to work independently with minimal training
