Join a collaborative outpatient team in Fayetteville, NC caring for geriatric and frail elderly patients. This permanent role covers clinical duties with shared on-call rotations and focuses on primary care, chronic disease management, preventive services, and transitional care.

What you’ll do

• Conduct comprehensive geriatric assessments and ongoing chronic disease management.
• Perform preventive care screenings and medication management.
• Support transitional care and discharge planning, including rounding and phone consults as needed.
• Share on-call responsibilities on a rotating basis.

Schedule and setting

• Location: Fayetteville, NC (outpatient)
• Clinical shift: Days (schedule to be determined)
• On-call: Rotational shared call (schedule to be determined)
• Coverage: Ongoing

Who you’ll work with

• A multidisciplinary team of physicians, nurses, therapists, and support staff in a collaborative environment.

Qualifications

• Active NC PA license
• NCCPA certification
• DEA registration
• BLS certification
• Minimum 2 years of PA experience (required)
• Experience with geriatric or frail elderly populations (preferred)
• Clean record highly preferred
