Family Medicine Physician — Value-Based Advanced Primary Care (Phoenix, AZ)

Location

• Phoenix, AZ (100% on-site). Additional opportunities in Gilbert, Peoria, and Scottsdale, AZ.

Role summary

• Provide comprehensive, patient-centered primary care within an integrated, team-based, value-based Advanced Primary Care practice.
• Serve a diverse patient population in a mission-driven organization focused on high-quality, compassionate care.

Compensation & benefits

• Salary: $240,000–$265,000 (DOE)

• CME: $2,500 annual allowance and 5 days PTO for CME

Required qualifications

• MD or DO from an accredited medical school
• Board Certified or Board Eligible in Family Medicine or Med-Peds
• Minimum of 5 years clinical experience preferred
• Current, unrestricted Arizona medical license
• Active DEA registration and Arizona fingerprint clearance card
• BLS certification
• Proficiency with EMR systems and basic computer skills
• Strong verbal and written communication skills and interpersonal abilities
• Demonstrated ability to work in team-based, value-based/integrated care models
• High standard of patient service and professionalism
