Family Medicine Physician — Value-Based Advanced Primary Care (Phoenix, AZ)

Location

• Phoenix, AZ (100% on-site). Additional openings in Gilbert, Peoria, and Scottsdale, AZ.

Position overview

• Join a Value-Based Advanced Primary Care team providing comprehensive primary care to a diverse patient population in an integrated, team-based environment.
• Full-time clinical role; focus on patient-centered care within an interdisciplinary practice.

Key responsibilities

• Provide comprehensive family medicine care to adults, children, and adolescents.
• Coordinate care with clinic team members and specialists in a value-based model.
• Manage chronic disease, acute care visits, preventive care, and care plans.
• Use EMR for documentation, ordering, and care coordination.
• Participate in quality metrics and population health initiatives.

Qualifications

• MD or DO from an accredited medical school.
• Board Certified or Board Eligible in Family Medicine or Med-Peds.
• Minimum of 5 years clinical experience preferred.
• Current, unrestricted Arizona medical license.
• Active DEA registration and Arizona fingerprint clearance card.
• BLS certification.
• Proficient with EMR systems and basic computer skills.
• Strong communication, interpersonal skills, and commitment to team-based care.

Compensation & benefits

• Salary: $240,000–$265,000 (DOE).
